VI Longitudinal CDI

The consent form (for EEG, play session, LENA) has a check box for parents to indicate whether they're open to follow-up studies. This information is entered into VIHI_contact_information.xlsx when paperwork is filed. VI families who consent for follow-up should be contacted for follow-up CDI scores at 6, 12, 18, 24, and 30 months (+- 1 if necessary).

VI_CDI_schedule.xlsx contains the dates for contacting families for follow-up CDIs. This file is password protected because it contains DOBs and SubjectNumbers.

When a new VI family signs the ELASI consent (2019-0199) and agrees to follow-up, add their name and DOB to VI_CDI_schedule.xlsxThis will generate the appropriate dates for follow-up CDIs and for contacting families for follow-up CDIs (one month before). Add google calendar events on VIHI for the new family on the all contact dates indicated (or nearest non-holiday weekday).

Create these reminders for all upcoming contact dates for new family.

The VIHI calendar also has a monthly reminder to check the spreadsheet for upcoming contact dates. Upcoming dates (within the next two months) are highlighted in yellow. Make sure that these families have been or will be contacted.

Contact families for follow-up using the following email template:

Hi PARENT,

I hope all is well in your neck of the woods! I'm reaching out because we have an opportunity for a follow-up study. 

This would involve filling out our two online surveys to give us a snapshot of how CHILD's vocabulary is developing.

The box should turn green once checked.

Create a calendar remind in VIHI to check in with the family 2 weeks after the initial email.
