PBS Survey

Step 1: Send out surveys to participants

Check to make sure that 4 Surveys are list

  1. Consent Form - R01 Consent form

  2. WebCDI - make a new one for each under PBS

  3. Demographics - Via Qualtrics

  4. Reading Survey - Via Qualtrics

Step 2: Schedule send out an email set 1.5 weeks in the future with the following text

“Check participant number _______ for survey completion”

Step 3: Check in on Surveys

1.5 weeks later if completed send gift card

If not completed, send follow up email
