Navigate to the ELASI print_forms folders, and print the relevant Consent, Payment forms, and the AV Release.

Get an iPad and pull up the relevant Qualtrics survey (make sure it's for the correct type of sensory impairment) - this contains both the demographics form and the HI_intake form.
the eye-tracking software/hardware is the same as that at the CI Center. Make sure the computers are set up properly. Prepare using this tutorial for sound-meaning, but make relevant tweaks.
